8.1% of AI Health Answers Use a Standard 'See a Doctor' Phrase

Someone who has slept badly for a month, or who feels low and cannot shake it, types the question into an AI engine late at night. The answer is calm, detailed and useful. In more than nine cases out of ten it carries none of the familiar lines such as "see a doctor" or "talk to a therapist".

People increasingly ask AI about symptoms, sleep problems and mental health. For clinics, telehealth services and health publishers, that means a standard line sending readers to a professional is not something to count on; the practical route to patients is being among the sources the AI reads.

A standard referral phrase is not the default. 8.1% of 4,248 AI health answers use one. The rate is highest for sleep questions, at 17.4%, and lowest for nutrition questions, at 0.7%.

On September 23, 2026 we asked ChatGPT, Gemini, Google AI Mode and Google AI Overviews the same 548 health questions, 137 each on general health, nutrition, mental health and sleep, twice each from a US location, and saved all 4,248 answers.

An answer counts as using a standard referral phrase when it tells the reader to see, consult, talk to or contact a doctor, healthcare provider, pharmacist, therapist, dietitian or other clinician, or to seek medical attention, call 911 or the 988 crisis line, or go to urgent care or the emergency room. Every figure is a share of all answers, with or without sources.

That is 344 of 4,248 answers. The other 91.9% answer the question without one of these standard lines about when a professional should get involved. The questions cover everyday health concerns, from symptoms and sleep to diet and mood, the kind people bring to a doctor every day.

A standard referral line matters because an AI engine cannot examine anyone, and it is the easiest kind of advice for a reader to spot and act on.

Google AI Mode uses one in 9.4% of its health answers, Google AI Overviews in 7.5% and ChatGPT in 5%.

Engines also point to care in other ways. In a companion Suff Digital study of the same answers, ChatGPT is the engine most likely to cite a government or medical source, so its answers often send the reader to an institution's own guidance.

The topic changes the picture. Sleep questions produce a standard referral phrase in 17.4% of answers, more than double the 7.3% for general health questions about conditions, symptoms and treatments. Many sleep questions touch on insomnia, sleep apnea and sleep medication, where a referral to a doctor or sleep specialist fits naturally.

Mental health is the result anyone working in behavioral health should note. 6.5% of AI answers to mental health questions use a standard phrase pointing to a therapist, counselor, psychiatrist or a crisis line such as 988, close to the 7.3% for general health questions.

Nutrition sits at the far end: 0.7% of nutrition answers use a standard phrase suggesting a dietitian or doctor, fewer than 1 in 100. Questions about diets, supplements and weight tend to be answered as lifestyle topics.

If you run a practice, clinic or telehealth service, do not assume AI engines will send patients your way. Your best route in is to be one of the sources AI reads: clear condition pages, plain-language explanations of when to seek care, and named clinicians who review the content. A page that states "see a doctor if symptoms last more than two weeks" gives an AI engine a sentence it can reuse.

Each specialty faces a different gap. Sleep has the highest referral rate, so being cited there can turn directly into bookings. Nutrition answers almost never use a standard referral phrase, so providers there need content that explains clearly when professional help is worthwhile.
